This form is required for clients and venues to receive a proper COI from our insurance provider.

Equipment Rental Application

This application must be filled out entirely and a proper COI must be sent sent in to be considered for direct rental approval. Some items we carry are not available for direct pickup and must be delivered and or setup by a member of our team. Please contact your sales person with any further questions.
